It may say that the office is closed due to holiday, for instance. Optionally, you could leave the phone off the hook so that it appears busy to the school or workplace.

If you are indeed confident in the ability of a friend or family member, one could mock a doctor's voice and take the call. This is the best option to ease suspicious, but it could easily prove to be a mistake if things take a turn for the worst.
