First, you could setup an answering machine on a phone number you have access to. You can record a personal message that sounds like a secretary's message to calling customers. It may say that the office is closed due to holiday, for instance. Optionally, you could leave the phone off the hook so that it appears busy to the school or workplace.

If you are indeed confident in the ability of a friend or family member, one could mock a doctor's voice and take the call. This is the best option to ease suspicious, but it could easily prove to be a mistake if things take a turn for the worst.

Whenever my Doctor writes an actual note, all he ever puts is "under my care". His philosophy is "if they really want to know (they usually don't care), they can call me."

This makes faking his notes extremely easy. 5 minutes with photoshop = change phone number and erase dates, print 50 copies, and set up the Asterisk IVR (Google it. You need to know a bit about Linux, but it's not too tricky to set up) from Hell on my Google Voice number, that's impossible to get through, but after 15 minutes of trying really badly worded menu options ("Press 4 for data inquiry" shoots data/fax modem noises down your phone and disconnects you) you can leave a voice mail message.

Using this system, I've quit two jobs by faking my death.
